IF OUR PHYSICIANS ARE OUT OF NETWORK WITH YOUR INSURANCE PLANS YOU MAY BE RESPONSIBLE FOR LARGER DEDUCTIBLES AND CO-INSURANCE. PLEASE CHECK WITH YOUR INSURANCE COMPANY FOR A LIST OF NETWORK PROVIDERS.
